Develop, implement, evaluate, and revise patient care coordination and case management programs while ensuring compliance with current standards of care, policies, and procedures.

Coordinate comprehensive patient care by collaborating with physicians, nurses, social workers, physical therapists, case managers, public health, and social services to address patient needs, manage clinic and follow-up visits, maintain health care requirements, and achieve timely, cost-effective outcomes.

Facilitate patient care conferences, provide interpretation support when applicable, conduct chart reviews, and document care coordination activities to monitor effectiveness, continuity of care, and compliance with documentation standards.

Collects relevant data for program evaluation, including service utilization, identified problems, and the status of resolutions.

Assesses and monitors patient satisfaction to support program improvement and enhance the quality and effectiveness of care coordination services.
