This clinic-based telehealth role focuses on delivering value-based care through a virtual platform while supporting fellow clinical team members through high-quality, team-based care.

Some Details About the Position:

• Clinic based onsite in Surprise, AZ
• Flexible scheduling: 4x10s or 5x8s shifts
• Medical Assistants, Front Office, and leadership support
• Support of an integrated system including specialists, urgent care, and more!

Essential Functions and Requirements:

• Provide comprehensive, high-quality care and support to established patients via telehealth visits
• A portion will be dedicated to inbox management support for team members
• DEA with prescriptive authority required
• FNP designation with AZ license eligibility
• Minimum (1) year of clinical experience as an FNP in a primary care setting
• Patient-focused, friendly, and willing to collaborate in a team environment
