Outpatient Physical Therapist

Provide outpatient physical therapy, evaluating patients and developing functional treatment plans with realistic goals. Monitor patient responses, document evaluations and progress, recommend next levels of care, and collaborate with patients, families, physicians, and care teams. Support quality improvement, supervise support staff, and contribute to professional education.
