Outpatient Vascular Nurse Practitioner

Community Care Physicians is seeking an Outpatient Vascular Nurse Practitioner or Physician Assistant to provide patient-centered care in the Capital Region of New York. In this role, you will assess patients with vascular concerns, obtain histories and perform examinations, review diagnostic findings, develop care plans, educate patients, and coordinate follow-up with physicians and clinical teams. Work collaboratively across a physician-owned multispecialty group committed to quality improvement, supportive teamwork, and lasting patient relationships. This outpatient opportunity offers access to strong support teams and growth across specialties and care centers. Applicants must hold an active New York NP or PA license and relevant board certification; vascular experience is preferred.
