Patient Care Coordinator / Liaison

Marketer coordinates home health and hospice services, building referral relationships across the Meridian area to connect patients with appropriate care. The role drives business growth and collaborates closely with physicians, hospitals, discharge planners, case managers, and community partners. Candidates bring healthcare sales and marketing experience and operate field-based, full-time duties.

Key Responsibilities:

• Build and maintain relationships with physicians, hospitals, case managers, discharge planners, skilled nursing facilities, assisted living facilities, and other referral sources.
• Visit hospitals, healthcare facilities, physician offices, and community organizations throughout the region.
• Promote Home Health and Hospice services and develop new referral opportunities.
• Manage and track referrals from initial contact through admission.
• Communicate with internal and external teams regarding referral status and admission needs.
• Assist with admissions, transfers, discharges, and insurance authorization processes.
• Maintain knowledge of Medicare, Medicaid, private insurance, and other payors.
• Participate in networking, community outreach, and healthcare marketing activities.
• Maintain accurate referral, census, and patient information.
• Provide exceptional customer service to patients, families, and healthcare partners.
• Maintain patient confidentiality and follow applicable healthcare regulations and organizational policies.
• Obtain Reptrax/VendorMate credentials for hospital access.

Qualifications:

• Healthcare sales and marketing experience required.
• Minimum 1 year of successful business development or marketing experience in healthcare, skilled nursing, assisted living, Home Health, Hospice, or related setting.
• Strong communication, interpersonal, networking, and relationship-building skills.
• Valid driver's license, auto liability insurance, and reliable transportation required.
• Ability to obtain Reptrax/VendorMate credentials for hospital access.
• Highly motivated, energetic, organized, and able to work independently in a field-based position.
• Knowledge of Medicare, Medicaid, private insurance, and other healthcare payors.

Preferred Skills:

• Associate Degree in Business, Marketing, Healthcare Administration, or related field preferred.
• Working knowledge of Medicare and Medicaid eligibility preferred.
• Clinical healthcare knowledge a plus.
