Physical Therapist Assistant

Physical Therapist Assistant needed for a 13-week travel assignment in a skilled nursing facility near San Jose. The role supports therapy teams, implements treatment plans, and documents progress. The position offers competitive weekly pay up to $2,100 based on experience.

Key Responsibilities:

Implement physical therapy treatment plans developed by the Physical Therapist

Assist patients in improving mobility, strength, balance, endurance, and functional independence

Provide therapeutic exercises, gait training, transfer training, and mobility interventions

Monitor patient progress and accurately document treatment outcomes

Educate patients and families on exercise programs, mobility techniques, and injury prevention

Collaborate with physicians, physical therapists, nursing staff, and other healthcare professionals

Maintain compliance with facility policies, state regulations, and professional practice standards

Support patients in achieving maximum functional mobility and independence

Maintain accurate and timely documentation within the electronic medical record (EMR)

Qualifications:

Associate Degree from an accredited Physical Therapist Assistant program

Current California Physical Therapist Assistant license

Current Basic Life Support certification

Strong communication, documentation, and patient care skills

Ability to work collaboratively within an interdisciplinary rehabilitation team

Preferred Skills:

Previous Skilled Nursing Facility experience
