Apply NowMethodist Healthcare at Home, part of SA Health’s network of home health agencies, is seeking a compassionate and patient-centered PRN Home Health Physical Therapist to join our Healthcare & Medical Services team. In this role, you will provide one-on-one physical therapy services to patients in their homes, helping them regain function, maintain independence, and improve their overall quality of life.
Working independently in the field, you will perform comprehensive assessments of strength, range of motion, balance, gait, pain, and functional mobility, as well as evaluate home safety and equipment needs. Based on your findings and physician orders, you will design and implement individualized treatment plans that may include therapeutic exercise, gait and stair training, transfer training, balance interventions, endurance training, and patient-specific home exercise programs.
You will deliver hands-on, evidence-based care, adapting interventions as patients progress, and providing clear education to patients, families, and caregivers on safe mobility techniques, proper use of assistive devices, fall prevention strategies, and long-term self-management. As a key member of an interdisciplinary team, you will collaborate closely with home health nurses, occupational and speech therapists, social workers, and physicians to coordinate care and support optimal outcomes.
Accurate, timely documentation in the electronic medical record is essential, including assessments, treatment plans, daily visit notes, and progress reports that comply with Medicare, state, federal, and agency standards. You may also provide guidance and supervision to physical therapist assistants as permitted by state regulations and organizational policy.
