PRN Home Health Physical Therapist

Methodist Healthcare at Home, part of SA Health’s network of home health agencies, is seeking a compassionate and professional PRN Home Health Physical Therapist to join our Healthcare & Medical Services team. In this flexible, per-visit role, you will deliver high-quality physical therapy services to adult and geriatric patients in the comfort of their homes, helping them regain function, improve safety, and maintain independence.

As a PRN Home Health Physical Therapist, you will perform comprehensive evaluations of patients’ functional mobility, strength, balance, pain, and home safety. Using your clinical judgment, you will develop and implement individualized treatment plans focused on improving functional outcomes, preventing falls, and promoting safe mobility. You will provide hands-on therapeutic interventions, including therapeutic exercise, gait training, neuromuscular re-education, transfer training, and patient-specific functional activities.

You will educate patients and their families or caregivers on home exercise programs, proper body mechanics, fall-prevention strategies, and safe use of assistive devices and durable medical equipment. Close collaboration with an interdisciplinary team—including physicians, nurses, occupational and speech therapists, and social workers—is essential to ensure coordinated, patient-centered care that reflects Methodist Healthcare at Home’s commitment to compassion and professionalism.

The PRN Home Health Physical Therapist will accurately document evaluations, visit notes, progress updates, and discharge summaries in accordance with regulatory, payer, and agency standards. You will monitor patient progress, reassess goals, and adjust treatment plans as needed, communicating any significant changes in condition to the clinical manager and referring provider. You will also participate in case conferences, follow agency policies and procedures, and uphold infection control and safety standards in the home setting.
