Methodist Healthcare at Home, part of SA Health’s comprehensive network of home health agencies, is seeking a compassionate and professional PRN Home Health Physical Therapist to join our Healthcare & Medical Services team. In this as‑needed role, you will deliver high-quality, patient-centered physical therapy care to individuals in their homes, helping them regain function, improve safety, and maintain independence.

You will be responsible for performing detailed physical therapy evaluations, developing individualized plans of care, and implementing evidence-based interventions such as therapeutic exercise, gait and balance training, transfer training, and fall-prevention programs. Working directly in the home setting, you will educate patients and family members on home exercise programs, safe mobility techniques, and the use of assistive devices, tailoring your approach to each patient’s unique needs and environment.

As an integral member of an interdisciplinary team, you will collaborate closely with physicians, nurses, occupational and speech therapists, social workers, and home health aides to coordinate and optimize patient care. You will monitor changes in patient status, communicate timely updates and recommendations to the care team, and adjust treatment plans to support the best possible functional outcomes.

This role demands meticulous, timely documentation of assessments, treatment sessions, progress notes, and discharge summaries in accordance with agency, state, and federal guidelines. You will participate in case conferences and quality initiatives that reflect our culture of compassion, professionalism, and commitment to clinical excellence.
