Position Overview Health Coast Behavioral Center is seeking a dedicated and licensed Physical Health Therapist to join our expanding team in Riverside, California. Work Environment This role requires an onsite presence for home care visits, where you will work closely with patients in their living environments. The office operates from Monday to Friday, from 9:00 AM to 5:00 PM, with some flexibility depending on patient needs. Key Responsibilities * Conduct comprehensive physical health assessments to identify patient needs and develop individualized treatment plans. * Implement evidence-based therapeutic interventions, such as therapeutic exercises and manual therapy techniques, to improve physical functionality and quality of life. * Educate patients and their families about physical health conditions, treatment strategies, and preventive care measures. * Monitor patient progress and modify treatment plans as necessary to ensure optimal outcomes. * Document patient assessments, interventions, and progress in compliance with legal and ethical standards. * Collaborate with other healthcare providers, including physicians and mental health professionals, to deliver coordinated care. * Participate in team meetings and case consultations to discuss patient care strategies and best practices. * Stay updated on the latest research and developments in physical therapy practices. Patient Population / Client Focus Your work will focus on a diverse patient population ranging from adolescents to older adults dealing with various physical health challenges. These may include chronic pain conditions, post-surgical rehabilitation, mobility issues, and those requiring preventive care to maintain their physical well-being. Qualifications * Hold a Bachelor's Degree in Physical Therapy from an accredited institution. * Possess a current and valid state license to practice as a Physical Therapist. * Demonstrated knowledge of physical therapy principles, treatment modalities, and evidence-based practices. * Strong assessment and evaluation skills to design personalized therapy plans. * Excellent communication skills to effectively educate and interact with patients and their families. * Proficiency in electronic health records (EHR) systems for accurate documentation and reporting. * Ability to work collaboratively with multi-disciplinary teams. Preferred Experience * Previous experience in outpatient care or home health settings addressing diverse patient needs. * Experience utilizing innovative therapeutic approaches that promote patient engagement and empowerment. * Familiarity with community resources and services that support patient recovery and health maintenance. Compensation and Benefits We offer a competitive salary range of USD 115,000 to 130,000 annually, commensurate with experience and qualifications.
