Work Environment The role is based onsite at our state-of-the-art rehabilitation center, where our professional staff work closely with patients in a supportive and encouraging atmosphere. Our facility is equipped with advanced therapeutic tools and resources, making it an ideal environment for both patients and therapists to thrive. You will work Monday to Friday from 9:00 AM to 5:00 PM, with some flexibility to accommodate the varying needs of patients. Key Responsibilities * Conduct thorough physical assessments to identify patients' physical health needs, enabling the formulation of individualized treatment plans that align with evidence-based practices. * Implement and evaluate therapeutic exercise programs designed to improve strength, mobility, and overall functional capacity of patients recovering from injuries, surgeries, or chronic conditions. * Collaborate with a multidisciplinary team, including physicians, psychologists, and mental health counselors, to provide integrated care that addresses both physical and psychological aspects of recovery. * Deliver education to patients and families about injury prevention, rehabilitation techniques, and lifestyle modifications that promote lasting health improvements. * Maintain meticulous records of patient progress in accordance with legal and ethical standards, ensuring confidentiality and compliance with healthcare regulations. * Participate in case discussions and quality improvement initiatives to enhance service delivery and patient outcomes. * Engage in continuing education opportunities to stay current with developments in physical therapy and rehabilitation. Patient Population / Client Focus The patients we serve have a wide array of needs, including recovery from orthopedic surgeries, management of chronic illnesses, and rehabilitation following accidents or injuries. As a Licensed Physical Health Therapist, you will work with individuals of varying ages and backgrounds, ensuring that treatments are culturally sensitive and tailored to each client's specific needs and life circumstances. Qualifications * Bachelor's Degree in Physical Therapy from an accredited institution. * Current and valid licensure as a Physical Therapist in the state of California. * In-depth knowledge of rehabilitative techniques, therapeutic modalities, and patient-centered care approaches. * Demonstrated ability to conduct comprehensive physical assessments and develop effective treatment plans. * Excellent communication and interpersonal skills, with a strong ability to build rapport with patients and their families. * Proficiency in electronic health record (EHR) systems and the integration of technology into treatment planning and documentation. * Commitment to ethical practice and professional development. Preferred Experience * Experience working in a rehabilitation center or outpatient facility, particularly with a focus on mental health and physical health integration. * Familiarity with a variety of therapeutic techniques, including but not limited to manual therapy, modalities, and exercise prescription. * Previous involvement in patient education and the provision of community resources to support physical health.
