Key Responsibilities * Conduct thorough assessments of patients' physical health and identify therapeutic needs to develop personalized treatment plans. * Implement evidence-based therapeutic interventions, including physical therapy exercises and rehabilitation programs tailored to individual patient profiles. * Regularly monitor and document patient progress, adjusting treatment plans as necessary to optimize recovery and well-being. * Collaborate with interdisciplinary teams, including psychologists, medical doctors, and social workers, to provide comprehensive care that comprehensively addresses the patient's needs. * Utilize tools and methodologies specific to physical health therapy, such as therapeutic modalities, manual therapy techniques, and patient education strategies. * Participate in team meetings and case reviews to discuss patient progress and share insights for continuous improvement in service delivery. * Engage in community outreach and education efforts regarding physical health awareness, rehabilitation benefits, and wellness strategies. Patient Population / Client Focus The patients you will serve include individuals seeking physical therapy for a variety of conditions, such as orthopedic injuries, chronic pain, neuro rehabilitation, and post-surgical recovery. Given the emphasis on mental health at Top Acres, you will also work with patients who have co-occurring mental health issues, providing a holistic approach to therapy that addresses both physical and psychological well-being. Qualifications * Active state licensure as a Physical Therapist. * Bachelor's Degree in Physical Therapy or a related field; advanced credentials are preferred. * Proven experience working in a clinical setting, preferably within behavioral health or multidisciplinary care environments. * Strong understanding of therapeutic modalities and techniques, and the ability to apply them effectively in treatment plans. * Excellent communication and interpersonal skills for effective patient interaction and interdisciplinary collaboration. Preferred Experience * A minimum of 2 years of experience in physical therapy, particularly in an outpatient or hospital setting. * Experience working with diverse populations and a commitment to culturally competent care. * Familiarity with electronic health record (EHR) systems for maintaining patient documentation and progress tracking.
